Please note that responses to this form are not monitored on a daily basis; however, we do monitor them each week. If you are aware of a hazard that is more critical, please call our Public Safety Dispatch Center at (801) 489-9421.

If you are reporting a fire or other public safety emergency that is in progress, call 911.

Again, this form is to report such things as fire hazards that are NON-emergency situations.
